Before Your Office Visit
- Before you can be registered as a patient you need to provide us with your personal
information and medical insurance information.You can provide us with this information
by any of three ways;
- over the phone
- in person at the time of the visit
- over the internet from our web site(Patient Registration).
- All insurance must be verified before your actual visit. If we cannot verify the
insurance or the coverage will not cover your type of office visit or procedure
we will notify you. If we are unable to verify the insurance coverage because an
insurance company may be closed as is sometimes the case over the weekend we have
a “Hold Payment Policy”. In other words we will ask you to pay for the visit and
we will return the payment minus applicable co payments when we are able to verify
coverage the following work day.
- All payments for co-payments or noncovered services are payable at the time of service.
- Our office will call to confirm your appointment. If you are not home and we need
to leave a message, we ask that you call our office at 631-587-6060 and dial extension
333. Leave your name, the name of the physician, the date and time of your appointment
and if you are confirming or canceling this appointment. If you need to reschedule,
call the office during normal business hours and Dial “0” for the operator.
Cancellation of Dermatology appointments with Dr. Panzer require 24 hours notice or there will be a $150.00 cancellation fee. This will be the case with “no shows” also.
